Would your patients benefit from peer support to help them improve their self-management skills?

HealthMakers self-management courses starting in April are now available for YOUR patients to book onto.

All patients who have a long-term physical health condition can benefit from attending our free 6 week course, and are available to anyone living in East Berkshire, with ANY physical long term health condition.
The friendly & interactive courses offer peer support and patients can work and learn with others to help improve their quality of life. 
Improved self-management can reduce the need for GP appointments, A&E visits or crisis with their condition.

Clients can self refer, however many will appreciate it you bringing it to their attention as their GP.

SLOUGH

Venue: Slough Aspire, 350 Edinburgh Avenue, Slough Trading Estate, Slough SL1 4TU

BRACKNELL

Venue: Bracknell Open Learning Centre, Rectory Lane, Easthampstead, Bracknell RG12 7GR
Date:   Commencing Wednesday 18^th April 2018
Time:   1pm – 4pm

MAIDENHEAD

Venue: Larchfield Community Centre, Desborough Park, Off Desborough Crescent, Maidenhead SL6 4BB
Date:   Commencing Thursday 19^th April 2018
Time:   1.30pm – 4.30pm
