Diabetes Structured Education

Diabetes Referral HUB

As of 1st October, all newly diagnosed diabetes patients should be referred to the Diabetes Referral HUB which will support them in finding the most appropriate structured education programme. The referral process is through DXS.

The referral hub will receive all referrals from within the East Berkshire CCG population. Referrals can be from:

  • GP Practices
  • Secondary Care
  • Patients (self-referrals)

Those referred can contact the hub directly on 0300 365 1234 to book the course, or the hub will contact the patient within 10 working days.

An appropriate level of coaching will be given to each patient to ensure that they:

  • understand the significance of their condition
  • understand the concept of the structured education and the significance of its use in helping
    them to manage their condition
  • are offered the range of options available including digital
  • are assisted in choosing the best option to suit their personal circumstances
  • are encouraged to take up the offer to attend a structured education course

The hub will manage the booking of patients into the best course for each individual and notify the
appropriate details to the relevant provider.

Education for people with Type 2 Diabetes

This is a course for people newly diagnosed with Type 2 Diabetes.  It consists of two 3 hour sessions covering the following information.

Changing Health is an internet based diabetes education programme for people with Type 2 Diabetes.

Education on diabetes, nutrition and exercise is accessible online via a computer or the Changing Health app.

A lifestyle coach will be available on the phone for support and to discuss simple lifestyle goals and how to achieve them
